Clinical Director

Position Requirements

1. Residency Requirement: Pennsylvania or New Jersey Resident, must be within 1.5-hour commute to the office from primary residence.

2. Pennsylvania Licensed Clinical Social Worker/Master’s degree in social work, clinical concentration

   1. Only applicants with active Pennsylvania LCSWs in good standing may apply

   2. Out-of-state LCSWs must first acquire a PA LCSW to apply

   3. Unfortunately, Pennsylvania LPCs & LMFTs are ineligible for the clinical director position. This is due to PA Medicaid laws prohibiting LAPCs and LAMFTs from providing psychotherapy services at group practices.

   4. Completes continuing education as required to maintain active PA LCSW

3. Credentialing with company in-network commercial insurances, including PA Medicaid

4. Able to work on-site at Perez Therapy locations 1 day/week and securely via telehealth

5. Able to provide clinical emergency on-call, as required

6. 5 years+ experience providing clinical social work as a psychotherapist

7. 5 to 10 years of management/supervisory experience in a behavioral health setting

8. Must be able to provide documented clinical licensure supervision for up to 6 LSWs and pre-clinically licensed professionals in the Commonwealth of Pennsylvania

9. Must not be prohibited from participating in the Medicaid/Medicare system as a provider

Clinical Position Requirements

2. Current resume, documenting at least 1 year of internship/employment experience as a psychotherapist.

3. Official graduate transcripts (LSWs must have 12 credits of clinical social work coursework).

4. Updated and attested CAQH profile/Data Summary with Perez Therapy, LLC listed as employer.

5. NPI-type 1 with appropriate matching taxonomy for your provider type.

6. Current active Pennsylvania license as an LSW, LCSW, LPC, LMFT or licensed psychologist.

7. Clear child abuse background check, FBI clearance and criminal history check.

8. Verification against federal and state exclusions lists, including, but not limited to:

   1. List of Excluded Individuals and Entities (LEIE)

   2. System for Award Management (SAM) (formerly Excluded Parties List System, EPLS)

   3. PA DHS Medicheck List

   4. NPPES (National Plan and Provider Enumeration System)

9. Copy of your social security card and active government-issued photo identification; 

10. For 1099 independent contractors only: proof of active professional liability with coverage of $1 million per occurrence and $3 million aggregate coverage for the practice of outpatient psychotherapy.
