Clinical Position Requirements

1. Current resume, documenting at least 1 year of internship/employment experience as a psychotherapist.

2. Official graduate transcripts sent the CEO’s email or our mailing address (LSWs must have 12 credits of clinical social work coursework).

3. Updated and attested CAQH profile/Data Summary with Perez Therapy, LLC listed as employer.

4. NPI-type 1 with appropriate matching taxonomy for your provider type.

5. Current active Pennsylvania license as an LSW, LCSW, LPC, LMFT or licensed psychologist.

6. Clear child abuse background check, FBI clearance and criminal history check.

7. Verification against federal and state exclusions lists, including, but not limited to:

   1. List of Excluded Individuals and Entities (LEIE)

   2. System for Award Management (SAM) (formerly Excluded Parties List System, EPLS)

   3. PA DHS Medicheck List

   4. NPPES (National Plan and Provider Enumeration System)

8. Copy of your social security card and active government-issued photo identification; 

9. For 1099 independent contractors only: proof of active professional liability with coverage of $1 million per occurrence and $3 million aggregate coverage for the practice of outpatient psychotherapy.
